Referral Process at Advanced Well-Being

At Advanced Well-Being, we make the referral process for supported accommodation simple and straightforward. Here’s how it works:

  1. Initial Consultation

    • Start with a phone call to discuss current vacancies and determine if the placement is suitable for the young person’s needs.
  2. Referral Form and Information

    • Complete a referral form and provide supporting documents about the young person.
  3. Assessment and Visit

    • We’ll assess the referral, and the young person may visit the accommodation to see if it feels like the right fit.
  4. Funding Agreement

    • Once everything is agreed upon, we’ll finalize the funding arrangements for the placement.
  5. Meeting the Key Worker

    • The young person will meet their key worker, who will explain house rules, routines, and expectations.
  6. Placement Date

    • A move-in date will be scheduled.
  7. Placement Planning Meeting

    • Within 72 hours of the placement, a planning meeting will be held to review goals and support needs.

Emergency Placements

Advanced Well-Being is also able to consider emergency placements based on availability.
